• Estimated Weekly Pay: $2,447 - $2,718\n
• On-Site Position: Reliable transportation required; winter driving conditions expected\n

\n\nLMSW Qualifications\n

\n
• Master’s Degree in Social Work (MSW)\n
• Active Vermont LMSW license\n
• Minimum 2 years of clinical experience in home health, hospice, or related healthcare settings\n
• BLS certification\n
• Strong assessment, counseling, and critical thinking skills\n
• Experience with end-of-life care planning and goals-of-care discussions\n
• Ability to drive in winter conditions; reliable vehicle with snow tires\n
• Complete work history and professional references required\n

\n\nSocial Worker Responsibilities\n

• Deliver psychosocial support, counseling, and resource coordination for patients and families facing terminal illness\n
• Conduct comprehensive assessments and create individualized care plans\n
• Collaborate with interdisciplinary healthcare teams to manage complex symptoms and ensure quality hospice care\n
• Educate patients and caregivers on hospice philosophy, symptom management, and available resources\n
• Document care and interventions using Net Smart EMR system\n
• Cross-cover adult home health, inpatient hospice (McClure Miller Respite House), and community hospice as needed\n
• Participate in weekend and holiday rotations\n
• Maintain professional appearance and adhere to dress code (business casual or non-institutional scrubs)\n
