Class: T-LPN Shift: 2Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse (LPN/LVN) delivers efficient and effective nursing care while promoting positive clinical outcomes and patient satisfaction. The LPN provides direct patient care, assists in nursing assessments, and supervises care provided by unlicensed staff — all while operating within the scope of practice defined by the State Nurse Practice Act.
ResponsibilitiesCollect, report, and document objective and subjective patient data.
\tObserve patient conditions and promptly report changes to the RN.
\tContribute to care planning and assist in developing and implementing individualized interventions.
\tAdminister medications and perform treatments per physician orders.
\tAccurately document care and communicate pertinent information to the RN and physician.
\tSupervise and assign duties to unlicensed staff as allowed by state regulations and company policy.
\tParticipate in interdisciplinary team meetings and discharge planning.
\tEnsure proper shift-to-shift communication and collaboration with other departments.
\tMaintain compliance with safety, infection control, and confidentiality policies.
\tUtilize PointClickCare (PCC) and other digital tools to document care accurately.
\tStay beyond scheduled shift as needed to meet patient or staffing needs.
\tGraduate of an approved school for Practical or Vocational Nursing.
\tCurrent and active LPN/LVN license in the State of New Mexico.
\tMinimum 1 year of clinical nursing experience required (rehab or geriatric experience preferred).
\tMust be able to read, write, and communicate effectively in English.
\tCurrent BLS/CPR certification .
\tAbility to maintain confidentiality, professionalism, and a service-oriented attitude.
\tMust provide verification of TB/TST per state requirements.
\tClassification: Grade 15 | FLSA: Nonexempt
\tMust comply with HIPAA and company compliance standards.
