Job Description

American Traveler is seeking an experienced RN Case Manager for a home hospice position requiring at least one year of hospice or home case management experience and EPIC proficiency.

Responsibilities

• Work as a Case Manager for home hospice patients in private residences, long-term care, and assisted living settings
• Day shift schedule with 5x8 hour shifts from 10:00 to 18:00 and every other weekend required
• Patient visits focus on symptom management and end-of-life support
• May be requested to float based on facility needs
• Interdisciplinary team (IDT) meetings occur weekly
• Must be comfortable working independently in home settings with support from interdisciplinary team
• Required to keep requested time off (RTO) to a minimum, with no more than 7 days total and no full calendar weeks off
• Previous permanent employees must have a 6-month separation before traveling
• Local candidates are accepted at the same rate
• Participation in IDT meetings and effective communication across care teams is expected

Requirements

• Active VA or compact RN license required
• Minimum 1 year of hospice or home case management RN experience within the last year
• EPIC charting experience required
• One supervisory reference is required for consideration
• Must provide a copy of driver's license and Social Security card at submission
