Job Description

American Traveler is seeking an experienced RN Case Manager for a Home Health position covering Vermont and New Hampshire, requiring a compact or VT/NH RN license and home health experience.

Responsibilities

• Work as a Case Manager in a home health setting across multiple towns in VT and NH
• Day shift, full-time position
• Provides care to patients in their homes within a defined service area including Bradford, Randolph, Springfield, White River Junction (VT) and Lebanon (NH)
• Responsible for developing, implementing, and revising individual patient care plans
• Assess, plan, coordinate, and evaluate patient care in collaboration with an interdisciplinary team
• Provide patient and family education and health counseling as needed
• Document all assessments and interventions according to agency guidelines
• Work both independently and as part of a supportive, cohesive care team
• No radius rule is in place; local candidates are accepted

Requirements

• Active VT, NH, or compact RN license required
• Graduate of an accredited school of nursing
• Minimum 2 years of recent home health RN experience required
• Current BLS certification required
