The Position: Social Worker

Employment status: Full time

Hours per two-week pay period: 72

Essential Job Functions:

• Works collaboratively with the team to address patients’ psycho-social needs in the inpatient and outpatient departments of the hospital.
• Assists individuals in meeting their needs related to the Social Determinants of Health.
• Works with the supervising Social Worker in the department.

Minimum Qualifications:

• Bachelor’s degree in Social Work or a related field or previous experience in discharge planning.

Education

• Bachelor’s degree in Social Work preferred.

Certifications and Licenses

• Licensure in Montana as a clinical Social Worker Preferred.

Position Summary:

Sidney Health Center is seeking an individual for the Social Work position to join our I CARE team in the hospital setting. Essential duties of the position include working with a team to provide for the psycho-social needs of the patients.
