What You’ll Do in This Role:
Make a Meaningful Impact as an RN Clinical Coordinator!
Join a mission-driven team where you’ll deliver compassionate, patient-centered care across home, clinic, and partner facilities. As an RN Clinical Coordinator, you’ll take the lead in managing complex care using PACE-specific assessments, collaborating with a dedicated Interdisciplinary Team to create and execute personalized care plans. Primary Responsibilities:
Facilitating Participant Integration
Integrate participants into the PACE of Southwest Michigan healthcare system
Conduct medication reviews, administer immunizations, and monitor acute/chronic conditions
Perform initial and periodic assessments in homes, facilities, the PACE center, and community settings
Use critical thinking to identify needs, prioritize care, interpret diagnostics, and assess clinical/ADL/IADL needs
Promptly report unusual findings to the PCP, supervisor, or Interdisciplinary Team (IDT)
Planning Care
Actively contribute to IDT meetings and care plan development across care settings
Collaborate with Home Care Coordinators to develop and monitor care plans for MAs and home health aides
Care Coordination
Administer treatments, medications, and procedures in line with care standards
Monitor medications with sensitivity to age-related needs
Educate participants on procedures and discharge plans; coordinate care transitions
Recognize and respond to emergency situations with advanced interventions
Ensure continuity of care by coordinating with healthcare personnel and supporting 24-hour care delivery
Collaborate on the coordination of Durable Medical Equipment (DME) and medical supplies
️ Evaluation & Documentation
Accurately document care, plan variations, and observations in medical records
Record any changes in care with rationale to support clinical decisions
Leadership
Support Quality Assurance and Performance Improvement (QAPI) efforts
Participate in the on-call nursing rotation per established procedures
✅ What You’ll Need:
Diploma or degree from an accredited School of Nursing or University
Current Registered Nurse (RN) license to practice in the State of Michigan
Minimum of 1 year of experience working with frail elderly or in long-term care
Previous home care experience preferred; long-term care experience considered
Strong clinical assessment skills and ability to manage complex medical situations
Ability to work independently and collaboratively within an Interdisciplinary Team (IDT)
Familiarity with PACE-specific care and regulatory standards is a plus
Excellent communication and interpersonal skills to engage with participants, families, and care teams
