In this role, you will have primary ownership over a panel of members with varying health conditions and needs. Your expertise in health management will guide the development and implementation of personalized care plans, enabling members to achieve their health goals. You will leverage data analytics and collaborate with a multidisciplinary team to ensure quality outcomes. This position is crucial for enhancing the lives of individuals by addressing their unique clinical needs.

Accountabilities

• Maintain oversight over a specified panel of members by performing ongoing assessments and identifying clinical interventions.
• Create comprehensive care plans addressing members' needs and conducting regular follow-ups for re-assessment.
• Ensure achievement of targeted goals related to patient outcomes and care plan realization.
• Document all activities in compliance with applicable regulatory and business process requirements.
• Stay updated with CMS, state, and local regulations pertaining to case management standards.

Requirements

• High School Diploma/GED required; Bachelor's Degree in Nursing preferred.
• Minimum of 7 years of experience in clinical, case management, or related fields.
• Current State of PA RN licensure or valid multi-state licensure required.
• Preferred certification in Case Management and experience with diverse populations.
• Strong analytical skills with proficiency in MS Excel and understanding of healthcare systems.
