Evaluates, assesses and plans nutrition care specific to outpatients of all ages. Additionally, provides nutrition information to staff, physicians, students and the community individually and through group programs. Reports directly to the Diabetes and Outpatient Nutrition Manager. Requires five to six years of formal training or education beyond high school (e.g., Masters Degree or five-year program). Registration required: Registered Dietitian - R.D. Licensure required: Licensed Dietitian - L.D. Requires experience working in a clinical setting (can include internship). At least 2 years experience in Diabetes education, nutrition, ability to work with a multidisciplinary team, and knowledge of growth and development of young and older adults in multicultural populations is preferred. Responsibilities include developing patient-centered treatment plans, completing comprehensive nutrition assessments, pursuing continuing education to develop and maintain DSME/T knowledge and skills, maintaining productivity to support patient access and office needs, managing workload and processes, managing insurance and charge requirements, completing hospital and departmental requirements, participating in hospital and/or departmental initiatives/special projects, taking responsibility for individual performance goals, and performing other duties as assigned. Physical demands include lifting and carrying/pushing less than 10 pounds regularly, repetitive motion, fine motor skills, sitting, speaking, occasional bending, reaching, squatting, walking, and exposure to blood borne and airborne pathogens. The noise level is usually quiet.
