The Licensed Vocational Nurse (LVN) plays a key role in delivering compassionate, high-quality nursing care within a community health center setting. This position supports the health care team by providing direct patient care, administering immunizations and medications, offering health education, and assisting in clinical and laboratory functions. The LVN ensures patients of all ages receive safe, timely, and effective care in alignment with TAN Healthcares mission of accessible, patient-centered health services.

Essential Job Responsibilities

• Deliver quality primary health care in accordance with nursing standards and TAN Healthcare protocols.
• Document patient care accurately and timely in the electronic health record (EHR).
• Provide patient and family education regarding diagnoses, treatment plans, medications, and preventive health practices.
• Triage walk-in patients and report concern promptly to the Nurse Practitioner or Physician.
• Coordinate referrals, medications, and follow-up care as directed.
• Administer immunizations, injections, wound care, and EKGs as ordered.
• Assist the VFC/ASN vaccine program.
• Assist with staff clinical competency checkoffs and training under the direction of the Director of Medical Services.
• Perform laboratory functions (following skill validation) and point-of-care testing per provider orders or standing orders.
• Accurately administer medications, following the five (5) rights of medication administration, and document per policy.
• Order and maintain medical supplies, PPE, and medication inventory; complete monthly reports and ensure safe storage.
  
• Ensure compliance with OSHA standards, universal precautions, and health center safety policies.
• Participate in continuing education, training, and professional development to remain current in best practices.
• Other duties as assigned.

Minimum Education, Qualifications and Experience

• Graduate of an accredited school of nursing with a Licensed Vocational Nurse (LVN) degree.
• Current, unrestricted license to practice as an LVN in the State of Texas.
• Current CPR for Healthcare Professionals certification.
• Minimum of one (1) year of direct patient care experience as an LVN (community health or primary care preferred).

Work Environment & Physical Requirements

• Ability to stand, walk, and move throughout the clinic for extended periods.
• Frequent bending, reaching, and occasional lifting of up to 25 pounds.
• Use of personal protective equipment (PPE) as required.
• Exposure to clinical environments include contact with patients, biological materials, and potentially infectious agents (with appropriate protections in place).
• Must be able to use computers, telephones, and other standard office/clinical equipment.
• Ability to remain calm and effective in a fast-paced, sometimes stressful environment.
