The Endocrinology Clinic provides a wide range of services for endocrine needs including subspecialty programs for diabetes, thyroid disease, and reproductive health patients. Our dedicated team of providers, nurses, social workers, pharmacists, and other health care givers care for the whole patient and use resources from the community to assist with ensuring the patient has what they need to improve their health. As a Certified Diabetes Education Specialist, you will provide direct patient education in individual or group settings following a specific curriculum developed for diabetes education. You will assess patients' physical, psychosocial, behavioral, and educational needs including self-management, SBGM, nutrition, physical activity, medication taking, and health maintenance labs, and interpret this information to identify individualized clinical and educational needs. You will teach technical skills, concepts of diabetes self-management, and facilitate behavior change for patients with Type 1, Type 2, gestational diabetes, and pre-diabetes. Documentation of assessments, interventions, evaluations, and follow-up will be consistent with departmental and ADA standards. You will individualize education presentations and materials to the patient and their circumstances, coordinate care with goal/outcome focus, and refer patients for additional services as needed. Patient responses and rationale for actions/decisions will be documented. You will maintain program content current with ADA standards and Fairview/M Physicians Diabetes Care. You will use organizational knowledge to meet immediate and critical patient needs, work collaboratively with healthcare team members including physicians, pharmacists, hospital and clinic staff, and demonstrate accountability for professional practice through performance assessment and peer review. You will support quality clinical initiatives related to diabetes care, be aware of measures and monitors in place, strive to help patients meet established goals, and adjust care approaches to reflect the developmental level of the population served.
